Visiting us for the first time? You can download your new patients form here. Just fill out the paperwork at home and then bring it with you along with your insurance card (if you have one), your driver’s license or photo ID. Or you can arrive 10 minutes prior to the scheduled appointment to fill out the paperwork in office.

Insurance and Payment Options

Our office accept cash, personal cheques, Visa, MasterCard and AmericanExpress. We also accept most dental health insurances, and we’ll file all your claims. We’ll make sure that you receive the maximum benefits available!
